1. Describe the current dentistry curriculum including detailed information on what is offered in the areas of dentistry and anesthesia.
A. Course content, skills taught, timing, hours spent and backgrounds of instructors.
B. Supplemental use of adjunct professors, online instruction, local shelters, community practices, and visiting wet labs.
C. Facilities, equipment, age.

2. Describe future program objectives, and enhancements to faculty, facility, equipment and curriculum to meet those goals.
A. Enhancements to dentistry/anesthesia curriculum including planned didactic, lab, co-op, and rotations (mandatory vs. elective)
a. Evidence that clinical time spent with equipment justifies either its cost to purchase, maintain and store or if periodic rentals/loaners for labs are a better option.
b. Note where the clinical curriculum is supported by practical application skills including scheduling/personnel, client education and marketing.

3. List resources needed including staff, facilities, equipment, and supplies (must have vs. nice to have; cost & timing).

4. Budget that includes sources/uses of funds and investment by university by MATCHING FUNDS (100%) including how the designated grant from the Foundation will be applied.

5. Startup costs versus ongoing operations, including equipment maintenance.

6. Performance measurements and outcomes; responsible party for tracking/reporting.

7. Letter of support signed by the Dean.
